

This session will outline the benefits of using My Health Record in hospital settings and at transitions of care to support medicine safety initiatives. Presenter from the Australian Digital Health Agency will compare how a new Pharmacist Shared Medicines List (PSML) document compares to other documents and overviews available in My Health Record and their relevance to hospital pharmacists. Guest speaker Chris Giles, Member of the Medication Safety Leadership Committee and Deputy Director (Pharmacy Practice Unit) at Icon Group will also reflect on complexities of implementation of new My Health Record documents in hospital systems and pharmacy service platforms.
