Care for Older Persons Colloquium
As Australia's population ages, caring for older people well depends on every member of the healthcare team. Pharmacists are playing an ever-greater role in the health and wellbeing of older Australians, working alongside nurses, physiotherapists, occupational therapists, dietitians, speech pathologists, social workers and other allied health professionals to deliver safe, coordinated and person-centred care.
Join AdPha for a full day of expert-led sessions exploring the clinical and practical challenges of caring for older people. The program covers changing pharmacist roles and managing inappropriate sexual behaviours, through to Parkinson's disease, traumatic falls and palliative care. These are complex areas where medicines, function, mobility, cognition and quality of life intersect, and where outcomes improve when disciplines understand each other's contribution.
The day combines specialist presentations with interactive case discussions, giving attendees the chance to work through real-world scenarios drawn from everyday practice. Each discussion highlights the medication considerations that shape care decisions, offering valuable insight for anyone involved in assessing, supporting or treating older people.
Delivered entirely online, this colloquium is a must for pharmacists working in aged care, hospital and community settings who want to deepen their expertise in geriatric care. Tiernan McDonough is an aged care onsite pharmacist and researcher, currently completing his PhD that aims to develop workplace-based training to upskill and better integrate pharmacists into aged care teams. As the PSA’s AMH Aged Care Pharmacist of the Year 2026, Tiernan is passionate about improving the quality of care for older persons through compassionate, evidence-based, interdisciplinary care models and practice.

Dr Reema Achal, Consultant Psychiatrist, SALHN Older Persons Mental Health
I am an Old Age Psychiatrist with a particular interest in dementia care and am also working towards an Advanced Certificate in Psychotherapy. I work at the Repat Health Precinct in a consultation-liaison capacity, with a primary focus on supporting subacute geriatric wards. My clinical practice focuses on supporting older adults with dementia and behavioural and psychological symptoms of dementia (BPSD), as well as managing a broad range of general psychiatric presentations.
Khoa Vo FANZCAP (PallCare), Senior Clinical Pharmacist, Canberra Health Services
Khoa Vo is a Consultant Pharmacist (FANZCAP, PallCare) with a specialist focus in palliative and end-of-life care. Originally from Vietnam, his early experiences witnessing inequities in access to pain relief for people with advanced illness inspired a career dedicated to improving the quality use of medicines at the end of life.
Khoa began his specialist palliative care journey in New Zealand, where he undertook academic and clinical training across hospice and palliative care settings and explored the role of pharmacists within multidisciplinary palliative care teams. Since relocating to Australia, he has practised as a hospital pharmacist with Canberra Health Services, including at Clare Holland House, the ACT’s specialist hospice service. His practice spans direct patient care, education, medication safety, policy development, and governance, alongside community-based medication management reviews.
Passionate about advancing specialist palliative care pharmacy practice, Khoa is committed to supporting pharmacists to optimise medicines use and improve outcomes for patients and families facing life-limiting illness.
Kathy Kirby, Director at Thrive Aged Care Consultants and Registered Nurse
Kathy is a highly experienced Registered Nurse with over 20 years of expertise across a wide range of healthcare and aged care settings. Her career spans acute, sub-acute, community, and aged care, giving her a comprehensive understanding of the diverse needs and challenges across sectors. With a strong foundation in clinical care, project management, and senior leadership roles, Kathy brings a unique blend of nursing expertise, management skills, and a passion for mentorship to her work.
Kate Lamont, Director at Thrive Aged Care Consultants and Registered Occupational Therapist
As Director of Thrive Aged Care Consultants, Kate supports aged care teams to build confidence and capability in dementia care. She is an Occupational Therapist with more than 20 years' experience across health, aged care and policy, specialising in supporting older people and people living with dementia in community, hospital and residential aged care settings.
A credentialed Positive Approach to Care (PAC) Champion Teacher, trained by internationally recognised OT and dementia expert Teepa Snow, Kate builds the skills of clinical and care staff. Her practical, evidence-based training helps teams understand the changes dementia brings, respond with empathy, and deliver care that preserves dignity and wellbeing.
Kate also brings deep expertise in dementia-enabling environmental design. She has worked with numerous providers to review their environments against industry standards and implement practical, sustainable improvements. Drawing on her background in policy, quality and project management, she partners with organisations to deliver lasting change across environments, systems and workforce practice.

Cancellation policy
- Membership rate is only applicable if you are a member at the time of the seminar. Should your membership lapse, or if you resign you will be invoiced for the difference in registration.
- Cancellations must be made in writing.
- Cancellations received before 6 weeks prior to the event will receive a full refund. Non-members will be charged a $50 handling fee.
- Cancellations received between 6 weeks and 1 week prior to the event will receive a 50% refund of the registration fee.
- Cancellations received less than a week prior to the event will NOT be refunded.
- AdPha reserve the right to cancel a seminar or make changes to the program
- If AdPha cancels or defers the event you will receive 100% of the registration fee but AdPha bears no responsibility for any costs incurred (such as flights, accommodation, travel or expenses) or loss of income.
- It is recommended that participants obtain adequate insurance cover for travel and accommodation.
- Transfer of registration – as an alternative to cancellation your registration may be transferred to another member of your organisation or another event (if the chosen seminar is a higher cost you will be subject to further charges). A request MUST be made in writing to the Federal Secretariat prior to the event.
- If you are unable to attend AdPha policy is that handouts can not be made available as ‘stand alone’ materials

Catering/Dietary requirements

- Whilst every care is taken to notify venues of dietary requirements, AdPha cannot guarantee completely allergy free meals, nor be held responsible for any adverse reaction. Any registrant who has notified AdPha of a dietary requirement should make themselves known to the seminar coordinator and/or venue staff upon their arrival.
- Some dietary requirements such as Kosher meals may incur an additional cost which is the responsilbility of the delegate. AdPha will notify you in advance.
Recording/Filming
- Recording and/or filming is not permitted by registrants. Please note this event may be recorded for the AdPha website and/or photos may be taken for use in AdPha resources and publications.
- Registrants are not permitted to record or film any part of AdPha events in any format including audio recording or video recording or through use of mobile devices for filming

- Numbers are capped so book early to avoid disappointment. Please note the only way to guarantee a space is to register online. Please ensure you register prior to booking your flights and accommodation.
- If you do not receive a confirmation email then you are not registered for the event and will not be able to be accommodated on the day of the event. If you are following up on registration please ensure you contact the Federal Secretariat at least a week prior to the event.
