WA celebrates its 2026 Branch Award Winners

AdPha's WA Branch has announced the winners of its 2026 Branch Awards, recognising outstanding pharmacists, technicians and teams whose work has strengthened patient care and advanced the profession across the state over the past year.

Achievement Award: To be announced!

Team Innovation Award: FSFHG Anticoagulation Stewardship Program

Team Leader: Breigh Ridley FANZCAP (Informtcs, Lead&Mgmt), Deputy Chief Pharmacist – Safety and Quality, Fiona Stanley Hospital

The Anticoagulation Stewardship Program at Fiona Stanley Fremantle Hospital Group has been recognised for its outstanding impact on venous thromboembolism (VTE) prevention and anticoagulation safety. Established in 2025 in response to rising hospital‑acquired VTE events and inconsistent risk assessment practices, the program introduced a structured, multidisciplinary stewardship model led by pharmacy and haematology. Its flagship initiative, VTE‑STOP, embedded consistent risk stratification, prophylaxis prescribing, reassessment, and discharge planning into routine workflows.

The team’s innovation extended beyond education and workflow redesign to include sustainable system‑wide strategies such as intern orientation, clinical champions, case‑based learning resources, and the pharmacist‑led “Thrombo Thursday” audit, which enabled rapid hospital‑wide compliance measurement. Their work also informed statewide anticoagulation chart redesign to integrate risk assessment and standardise heparin prescribing. Together, these initiatives have significantly strengthened VTE prevention, improved prescribing safety, and established a robust stewardship framework that is now being rolled out across additional specialties. The Anticoagulation Stewardship Program exemplifies multidisciplinary leadership, practical innovation and measurable impact.

Congratulations to all of this year's WA Branch Award winners, and thank you to everyone who took the time to submit a nomination.
