Toni Howell, Medication Safety and Strategy Pharmacist, St Vincent’s Hospital, Melbourne, Victoria

Toni is the Medication Safety and Strategy Pharmacist at St Vincent’s Hospital Melbourne and also works for the Australian Council on Healthcare Standards as an Assessor. Toni gives some university lectures to Pharmacy Students, and occasionally provides medication safety consulting services. She has held a number of clinical and management positions in Australia in both public and private hospital, including as a Director of Pharmacy, been employed on secondment to the Victorian Department of Health, and has worked overseas as a primary health care pharmacist in Botswana. She has received several awards for medication safety, including the Melbourne Health Celebrating Excellence: Quality and Safety Award and the St Vincent’s Health Australia Innovation and Excellence Award for Person Centred Care. Toni has been a member of AdPha for longer than her youthful bio photo suggests, but she can’t update the photo because of a long-standing joke with a pharmacist friend from university.
Nam-anh Nguyen FANZCAP (MedsMgmt, MedSafety) , Medicines Management Pharmacist, WA Country Health Service, Perth, WA

Nam-Anh Nguyen works as the Medicines Management Pharmacist for WA Country Health Service. Nam-Anh leads the best practice of medicines management and the stewardship of medication safety for more than 100 facilities providing varied services to half a million people across 2.5 million km2. Her strengths are balance and agility. She believes that we cannot focus on safer systems without consideration of efficiency, quality and equity, nor can we stagnate, ignoring the challenges/changes within healthcare and communities. Nam-Anh finds it deeply rewarding to facilitate systems changes which tangibly boost the confidence of staff working with medicines, and the confidence of patients to manage their own medicines, and is proud to be an impactful leader across WA and nationally.
Wendy Ewing FANZCAP (MedSafety), Deputy Director of Pharmacy – Quality, Monash Health, Clayton, Vic

Wendy Ewing is the Deputy Director of Pharmacy – Quality at Monash Health, Victoria. In this role Wendy oversees medication safety and medication stewardship services across Monash Health, the largest public health service in Victoria and is responsible for the quality and safety activities of the Pharmacy Department. Wendy chairs the Victorian Therapeutics Advisory Group (VicTAG) Quality Use of Medicines Group and is a member of the VicTAG Management Committee. She is a founding member of the Advanced Pharmacy Australia (AdPha) Medication Safety Leadership Committee and passionately advocates for patient safety through collaboration, leadership and continuous improvement. Wendy’s professional interests other than medication safety include critical care, pharmacy technician development, interprofessional education and workforce wellbeing.
Linda Graudins, Medication Safety Consultant Pharmacist, Melbourne, Vic

Linda is proud to say that she has been an AdPha member and hospital pharmacist for several decades, working in Sydney and Melbourne, with a short stint in Boston. As a member of the Medication Safety Leadership Committee, she was the GRIT medication safety editor and is a co-author of the updated Medication Safety Standards. Illustrating the ever-expanding options available in hospital pharmacy, Linda established the first pharmacy service at the Prince of Wales Hospital Emergency Department and, after completing research with Dr Madlen Gazarian in decreasing paediatric medication errors, was the first QUM/ Medication Safety pharmacist at the Sydney Children’s Hospital. From 2010 to 2025 Linda was the senior medication safety pharmacist at Alfred Health, Melbourne, leading the network ADR Review Committee and the Medication Safety program. Linda is currently a regional editor for the International Journal for Quality in Healthcare and an invited lecturer for both Latvian pharmacy faculties.
Claire Fitzgerald FANZCAP (Lead&Mgmt, MedSafety), Head of Quality and Medication Safety, Slade Pharmacy, Sydney, NSW

Claire FitzGerald is the Head of Quality and Medication Safety at Slade Pharmacy, bringing over 20 years of experience in hospital pharmacy across both public and private sectors. Her expertise spans operational management, clinical research, medication safety, quality improvement, education and training, compounding, stewardship, clinical trials, oncology, and clinical pharmacy services. Claire leads the Quality and Medication Safety Unit (QMSU), which oversees clinical governance and medication safety across more than 70 sites within Icon Group. The QMSU also supports hospital partners in achieving NSQHS accreditation and advancing medication safety practices. She is an active contributor to the profession, currently serving on two Advanced Pharmacy Australia (AdPha) Specialty Practice Leadership Committees—Medication Safety and Leadership & Management (Chair). Claire is deeply passionate about medication and patient safety, advocating for the evolving role of pharmacists in hospital and community settings. She champions the quality use of medicines and supports extended scope roles for pharmacists to enhance patient outcomes.
