Leading voices come together to enhance pharmacy programs
AdPha’s Voices in Agreement series gathers leaders from aged care, transitions of care and geriatric medicine to improve pharmacy programs.
Advanced Pharmacy Australia (AdPha) today welcomed leaders from across the aged care industry sector to its first of three virtual sessions as part of its co-governance role toward a new pharmacy programs agreement which complements last year’s 8th Community Pharmacy Agreement (8CPA).
The Voices in Agreement series the series begins with an Industry Medication Advisory Committee (MAC), followed by a GP and Consumer Consultation, and a consultation gathering elected leaders from AdPha’s Transitions of Care and Primary Care (TCPC) and Geriatric Medicine Specialty Practice Leadership Committees.
AdPha President Tom Simpson FANZCAP (Lead&Mgmt) says aged care sector leaders share a patient-centred focus on evidence that is native to Australian hospital pharmacy.
‘This series is a valuable opportunity to work together to enhance these crucial pharmacy programs. It’s only through the latest research and evidence that we can ensure these programs are appropriately targeted to our most vulnerable Australians first and foremost, maximising health outcomes and keeping more people well and out of hospital.
‘As Australia’s peak body of hospital pharmacy for more than 80 years, we look forward to distilling expert aged care insights in the first of our Voices in Agreement series, complementing engagement led by the Pharmaceutical Society of Australia.
‘AdPha is committed to helping shape pharmacy programs that better support the safe and effective care of older Australians.’
AdPha Chief Executive Kristin Michaels FAICD says the series will reflect the reality of the care continuum in Australia and the importance of safe, innovative and evidence-based models of pharmacy care.
‘In Australia we have complex and transitional care models, requiring clear communication, robust clinical pharmacy standards and strong collaboration.
Pharmacy programs – including the Residential Medication Management Review (RMMR), Quality Use of Medicines, Home Medicines Review (HMR) and Aged Care On-site Pharmacist (ACOP) programs – are crucial to providing consistent, interdisciplinary care that wraps around the individual.
‘This is our guiding light as we work toward improving access and outcomes for Australians.’
